MCS provides medical marijuana certifications to patients with certain qualifying medical conditions. We cater to those for whom privacy and discretion are of paramount concern. Our team of medical professionals guide patients through every step of the process. We help our patients obtain the required medical records, schedule a physical exam, and prepare the State application for a medical marijuana card. We are committed to completing all of the necessary steps for obtaining a medical marijuana card before our patients leave the office.
Step 1 Medical Record
We are happy to help our patients prepare and submit their ADHS application for a medical marijuana certification. All medical records must be current and must support a diagnosis for at least one of the qualifying medical conditions listed in A.R.S. § 36-2801. If you would like us to obtain your medical records on your behalf, please complete our Medical Release Form http://www.medcertserv.com/clients.htm#jump, otherwise fax your records to our office at (480) 383-6996 or mail your records to our office at: 2211 E. Highland Ave., Suite 110 Phoenix, Az 85260. We will begin the screening process immediately upon receiving your medical records. After determining that there is sufficient support in your medical records to meet the qualifying criteria, we will contact you to schedule an appointment for a physical exam.

Step 2 Appointment
We ask that you arrive 10 minutes early for your appointment so that you will have plenty of time to complete the forms necessary for Check-In. We require payment in full for our services at the time of check-in. Our fee is $150 and we accept cash, check, money orders, credit, and debit as payment. We offer a 10% discount for those patients who pay in advance of their scheduled appointment either online through our website or over the telephone.*
After checking in, you will meet with a MCS physician for a standard medical Physical Exam. The physician will either diagnose or confirm a diagnosis of the qualifying condition that is supported by your medical records. He or she will also discuss some of the potential risks and benefits of using medical marijuana. Once the physician certifies that you are likely to receive some benefit from the use of medical marijuana to treat or alleviate your condition, you will move onto the third and final step of Application Preparation.
*MCS will contact you by both email and telephone after the medical record screen, at which point you will have the opportunity to pay in advance.

Step 3 Medical Card
We are happy to help our patients prepare and submit their ADHS application for a medical marijuana card. This is a value-added service that we offer as a convenience to our patients. Of course, you are also welcome to submit your own application if you wish. You should also be aware that ADHS charges a $150 application fee that is payable by credit or debit only. The application must be submitted electronically through the ADHS Medical Marijuana website.
There are several required attachments to the application, such as the Physician Certification Form, a digital photograph, an acceptable form of identification, and a signed attestation form. We will convert these completed documents to an electronic format and attach them to the application for submission before you leave our office. It will take about ten business days for you to receive a medical marijuana card from ADHS. If any further information is required for your application, we will work closely with you to provide whatever is necessary.
